Welcome patrons! Attached below you will find a "checklist" for what you will need to register your child(ren). Also attached are forms for registration, health screening tool, and health assessment/sports physical. Please complete the attached forms before you arrive at Central Registration. If you have questions please call us at 684-5137, 684-5135 or 684-5138.

• Please complete a separate Registration Form for each child.
• For children 5th grade and younger, a current immunization/shot record must be presented when turning in registration form.
• For the Health Screening Form: Please complete a separate one for each child. Be sure to answer all the questions and sections and sign/date at the bottom.
• For the Health Assessment/Sports Physical: Please complete the front page and have doctors office complete and sign the second page. If your child has a recent physical, this will suffice. You can then complete the first page of the Health Assessment and bring the physical form with you.
• A current Sports Physical is needed to enroll in any sports activity. (Physical must be valid throughout entire season.)
